This form is intended for returning students to secure an advance spot in Nicole’s lesson schedule. Please read and complete the form below to register for the new term.

There is a non-refundable $35 registration fee which will be applied as a credit toward your first lesson. Once your form has been processed, you will receive an invoice payable via bank transfer which is due upon receipt. Scheduling forms & dates for fall term will be made available mid-August. Pre-registered students who fail to arrange and confirm their lesson schedule after the first full week of the the term will risk forfeiting their reserved slot to a new student.

Lesson Format

Choose between a virtual or hybrid lesson option below. In-studio time slots are limited. Hybrid lessons will be in person except in the event of student or teacher illness, or if the student prefers to meet virtually. As singing is an activity with a higher risk of exposure, we are implementing a few COVID safety policies to minimize that risk as much as possible. All lessons immediately after vacations or school breaks will be virtual, In-person lessons will resume the following week.

By submitting this form, I hereby give permission for the Student listed above to attend and participate in voice lessons or other events affiliated with Ferretti Vocal Studio (FVS). I agree that any audio, photo or video material recorded by FVS may be used for promotional purposes. I hereby release and discharge FVS, Nicole Ferretti, from any and all liability for any action, claim or damage that may arise as a result of the Student’s participation in voice lessons or other related events. I have read and agree to be bound by the policies outlined on the policies page provided above.
